    To be eligible for membership, you must work for a Lufthansa Group company, be a relative of an employee who works for a Lufthansa Group company, or be a retiree from a Lufthansa Group company. See list* at bottom of page and contact us if you have any questions regarding membership eligibility.

    Instructions:

    Please print out and complete the following:

    • New Account Form
    • W-9 Form
    • Copy of government issued photo ID (Drivers License or Passport)
    • Transfer on Death Agreement if you wish to designate a beneficiary
    • IRA Form (if applicable)

    Please return all forms to: Lufthansa Federal Credit Union, 1065 Old Country Rd - Suite 215, Westbury, NY 11590. You may also email the forms back to us. Also, make sure you read the "Truth in Savings" provisions.

    • Transfer on Death Agreement - Please complete if you wish to name a beneficiary for your account.

    • Truth in Savings - You must read this before opening an account with the credit union

    • 5305-A Form - Please complete this form ONLY if requesting a TRADITIONAL IRA account.
    • 5305-RA Form - Please complete this form ONLY if requesting a ROTH IRA account.
    • 5305-EA Form - Please complete this form ONLY if requesting an EDUCATIONAL IRA account.
    • IRA Beneficiary Form - Please complete if you wish to name a beneficiary for your IRA account.
